Member name change requests.

Been getting a fair number of these lately..

Please note that this is a function that I have to do myself, and have chosen to make this a perk of paid members only. Sorry about this, but I had to put a price tag on my time for such things. Otherwise I would be doing several of them per day, eating up my already greatly diminished "optional" time allotment.
