I don't think its terribly sick either. It is rather thin, but not dangerously. and yes, the puffed neck is from eating. You should probably just mention that he looks skinny and they should up the meal rations a little. and also maybe mention that it could go for a good bath and a good drink. Just be polite about it, and the owners should realize that you just have the best interest of the animal at heart and you're good herp keepers and are just trying to offer some advice.
